New Patients
What To Expect
Initial visits generally consist of an exam, therapies, and an adjustment (manual, low-force, and/or instrument). The average visit lasts 30-45 minutes. Please bring your driver's license and insurance card and arrive 15 minutes early to complete paperwork. Alternatively, you can download and fill out the form below and attach it to our message form, or print it off, fill it out, and bring it with you to your appointment.

We accept most insurances, subject to deductibles and co-pays, including:

There is a set fee for self-pay patients.
